NEW SERVICE

Forms:

Application checklist for New Installation Service that needs to be tapped into line:

  • Property Deed
  • Photo ID
  • Name ID (ex.) Birth Certificate, Social Security Card, or any legal document with name on it
  • A 335 E form for sewer installation
  • If service is in Scott County you will need a new Development form as well (this is a 911 address)
  • The customer applying must be the one to come in and bring paperwork and money before service is connected.

Application checklist for New Services already having a meter installed:

 Homeowners must come in and bring the following:

  • Photo ID
  • Name ID (ex.) Birth Certificate, Social Security Card, or any legal document with name on it
  • The customer applying must be the one to come in and bring the paperwork and money before service is connected.

 Renters must come in and bring the following:

  • Photo ID
  • Name ID (ex.) Birth Certificate, Social Security Card, or any legal document with name on it
  • The customer applying must be the one to come in and bring the paperwork and money before service is connected.

Termination of services:

  •  Customer may call or come in to discontinue service at an existing service (you will be asked a question to verify you are the owner of the account)
  • Account number and name of service will be needed
  • A forwarding address will be needed
  • A definite date to terminate service (excluding weekends and holidays)
